Abstract Background: Real-world, primary data on the treatment of psoriasis are scarce, especially concerning the role of soluble biomarkers as outcome predictors. Objective: The authors evaluated the utility of Th1/Th17 serum cytokines along with clinical characteristics as predictors of drug survival in the treatment of psoriasis. Methods: The authors consecutively included participants with moderate to severe psoriasis who were followed up for 6 years. Baseline interferon-α, tumor necrosis factor-α, and inter-leukin (IL)-2, IL-4, IL-6, IL-10, and IL-17A were measured using a cytometric bead array; clinical data were assessed. The authors calculated hazard ratios (HRs) for drug survival using a Cox proportional hazards model. Results: The authors included 262 patients, most of whom used systemic immunosuppressants or biologics. In the multivariate model, poor quality of life measured by the Dermatology Life Quality Index (HR = 1.04; 95% CI 1.01-1.07; p = 0.012) and elevated baseline IL-6 (HR = 1.99; 95% CI 1.29-3.08; p = 0.002) were associated with treatment interruption. Study limitations: The main limitation of any cohort study is the presence of confounders that could not be detected in clinical evaluation. Conclusions: Poor quality of life and elevated baseline serum IL-6 level predicted treatment interruption in patients with moderate to severe psoriasis. Although IL-6 is not the most important mediator of the inflammatory pathway in the skin environment, it is an interesting biomarker candidate for predicting psoriasis treatment response.

ABSTRACT

Objective: to map scientific knowledge on nursing students' attitudes and perceptions regarding ageism. Method: scoping review according to the recommendations of the Joanna Briggs Institute. The study question was: What are scientific evidence available on the attitudes and perceptions of nursing students regarding ageism? The search was conducted in 12 databases using the Rayyan application and the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews. The studies were selected by two reviewers using a data extraction tool. Results: of the 4,595 files, 46 that were published between 1984 and 2022 were selected, and the quantitative method was the most used. The most commonly used instrument was the Kogan's Attitudes Toward Old People Scale. Positive, negative, mixed, neutral, and inconclusive attitudes and perceptions were identified. Conclusion: attitudes and perceptions about ageism are diverse and not conclusive. Future intervention studies are recommended to detect changes in the behavior of nursing students in the face of ageism.


Objetivo: mapear el conocimiento científico sobre las actitudes y percepciones de los estudiantes de enfermería respecto al edadismo. Método: La pregunta del estudio fue: ¿Qué evidencia científica está disponible sobre las actitudes y percepciones de los estudiantes de enfermería respecto al edadismo? La búsqueda se realizó en 12 bases de datos utilizando la aplicación Rayyan y la Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews . Los estudios fueron seleccionados por dos revisores mediante una herramienta de extracción de datos. Resultados: de los 4.595 artículos, se seleccionaron 46 publicados entre 1984 y 2022, siendo el método cuantitativo el más utilizado. La herramienta más utilizada fue el Kogan's Attitudes Toward Old People Scale . Se identificaron actitudes y percepciones positivas, negativas, mixtas, neutras y no concluyentes. Conclusión: las actitudes y percepciones sobre la discriminación por edad son diversas y no concluyentes. Se recomiendan futuros estudios de intervención para detectar cambios en el comportamiento de los estudiantes de enfermería ante el edadismo.

A busca foi realizada em 12 bases de dados utilizando o aplicativo Rayyan e o Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews . Realizou-se a seleção dos estudos por dois revisores, com uso de um instrumento para a extração dos dados. Resultados: dos 4.595 arquivos, selecionaram-se 46 publicados entre 1984 e 2022, sendo que a maioria utilizou o método quantitativo. O instrumento mais utilizado foi o Kogan's Attitudes Toward Old People Scale e os estudos mostraram que os estudantes apresentaram atitudes e percepções positivas, negativas, mistas, neutras e não conclusivas. Conclusão: as atitudes e percepções sobre idadismo são diversificadas e não conclusivas. Recomenda-se estudos futuros de intervenção para detecção de mudanças de comportamento dos estudantes de enfermagem frente ao idadismo.

Abstract Objective: To investigate the effects of different physical exercise programs and polymorphisms of the FTO (fat mass and obesity-associated gene) on body composition and cardiovascular risk factors in adolescents with overweight and obesity. Methods: A randomized, parallel, double-blind clinical trial consisting of the adolescent overweight from the state public network, in a simple representative random sample, who participated in an aerobic exercise or weight training intervention for 10 weeks. Anthropometry, body composition, biochemical markers, sexual maturation, and rs9939609 polymorphism in the FTO gene were assessed. 347 adolescents had their characterization of nutritional status. 72 individuals with overweight and obesity were invited to participate. 39 remained for the start of the program and were randomly allocated to both types of intervention. In the end, 26 subjects participated in the intervention programs, with 12 and 14 in the aerobic and weight training programs, respectively. Results: Heterozygous and homozygous bearers of risk allele A participating in the aerobic program showed improvements in glycemia (p = 0.002) and total cholesterol (p = 0.023) and a reduction in body fat mass (p = 0.041). The weight training program reduced glycemia in patients with the risk allele A (p = 0.027). Cameron's stage four sexual maturation participants were 2.1 times more likely to improve their body fat (CI = 1.31-3.39). Conclusion: Aerobic exercises produced exclusively a significant decrease in fat mass and total cholesterol in patients with risk allele A. Distinct physical exercise programs may cause diverse changes in risk variables related to the health of adolescents.

ABSTRACT

O uso massivo dos agrotóxicos nas lavouras deu-se a partir de 1950 com a "Revolução Verde", como resultado da busca por aumento da produtividade e modernização dos campos agrícolas. Diante disso, na década de 1960, foi criado o Programa Nacional de Defensivos Agrícolas (PNDA), que veio para facilitar a introdução dos agroquímicos, colaborando para que, a partir de 2008, o Brasil passasse a ser o país com maiores percentuais de uso destes produtos. Essas substâncias geram efeitos deletérios sobre a resposta imune dos indivíduos expostos, principalmente relacionada aos macrófagos, células B, T e NK. Isso afeta a capacidade de fagocitose, apresentação de antígenos e produção de anticorpos, além de induzir a geração de radicais livres de oxigênio e disfunção mitocondrial, resultando em estresse oxidativo e danos ao DNA celular, apoptose em excesso, mutação no ciclo celular, desordem de regulação e, consequentemente, imunodeficiência. Dessa forma, o desenvolvimento de doenças imunomediadas, como asma e doença pulmonar obstrutiva crônica (DPOC), está estreitamente ligado aos agrotóxicos, uma vez que esses variados mecanismos de toxicidade ao sistema imune induzem, dentre outras, manifestações respiratórias, tais como tosse, sibilo, irritação e inflamação. Além disso, estes pesticidas estão relacionados com doenças não imunomediadas ao alterar a função normal dos hormônios da tireoide, andrógenos e estrógenos. A fim de avaliar estes impactos, o presente estudo consiste em uma revisão integrativa da literatura e, diante da crescente utilização descontrolada dos agrotóxicos, assume grande relevância, refletindo a necessidade de maior atuação da vigilância epidemiológica, ambiental e da saúde do trabalhador.


Beginning in the 1950s, massive pesticide use began in what is called the "Green Revolution", a quest for increased agricultural productivity and modernization. In the 1960s, the Brazilian National Program of Agricultural Defense was created to facilitate the introduction of agrochemicals, leading the country to become one of the world's largest pesticide users by 2008. These substances have deleterious effects on the immune response of exposed individuals, mainly related to macrophages and B, T, and NK cells. This affects phagocytosis and antigen and antibody production, inducing production of oxygen free radicals and mitochondrial dysfunction, which results in oxidative stress and cellular DNA damage, excess apoptosis, cell cycle mutations, regulatory disorders, and, consequently, immunodeficiency. Thus, the development of immune-mediated diseases, such as asthma and chronic obstructive pulmonary disease (COPD), is closely linked to pesticides, since these varied mechanisms of toxicity to the immune system induce respiratory manifestations, such as cough, wheezing, irritation and inflammation. Pesticide use is also related to non-immune-mediated diseases because exposure alters the normal function of thyroid hormones, androgens, and estrogens. To evaluate their impact, the present study performed an integrative review of the literature, which, due to the growing and uncontrolled use of pesticides, is of great relevance and demonstrates the need for greater epidemiological, environmental, and worker health surveillance.

ABSTRACT

Abstract Varicella is a common viral infection in childhood and usually has a benign evolution. However, the evolution can be severe in immunosuppressed children. Vaccination can prevent its occurrence and especially the development of severe and/or complicated cases. Methotrexate is a systemic therapeutic option for many inflammatory diseases, and its immunosuppressive action exposes its users to a higher susceptibility to infection. The present report describes a case of severe varicella infection in a child immunosuppressed with methotrexate.

ABSTRACT

This study aimed to verify the efficiency of multilines in reducing blast progress and their potential benefits to phenotypic stability in rice. The experiments were conducted in the 2016/17 and 2017/18 agricultural years. A randomized block design was performed with three replications, evaluating 12 lines and a multiline, which consisted of five lines from the Cultivation and Use Value (CUV) test. The multiline presented an estimated grain yield above the average of experiments of around seven bags ha-1 and superior performance in early flowering, justifying the high phenotypic stability for these characters. In this case, the line selection for composing the multiline was favorable and efficient, highlighted by a higher agronomic performance than most lines of the CUV test. The multiline is an adequate strategy to provide higher phenotypic stability and reduce blast progress in the field.

ABSTRACT

O objetivo do presente trabalho foi identificar os elementos que compõem as representações sociais de estudantes universitários sobre a participação de mulheres no contexto de trabalho, tendo em vista a orientação sexual dessas mulheres. Para isso, 177 universitários responderam o Teste de Associação Livre de Palavras (TALP), com os termos indutores "mulheres no contexto de trabalho" e "lésbicas no contexto de trabalho". Os dados foram analisados por meio da Análise Prototípica, realizada pelo programa Iramuteq. Para as mulheres em geral, os resultados indicam o uso de palavras com conotações aparentemente positivas, mas que reforçam estereótipos de gênero, apontando funções socialmente definidas para mulheres. Para as lésbicas, os resultados centralizam o conteúdo no preconceito sofrido por essas enquanto membros da comunidade LGBT. (AU)


The objective of the present study was to identify the elements that compose the social representations of university students about the participation of women in the work context, considering the sexual orientation of these women. To that end, 177 university students responded to the Free Word Association Test (TALP), with the terms "women in the work context" and "lesbians in the work context". The data were analyzed through the Prototypic Analysis, performed by the Iramuteq program. For women in general, the results indicate the use of words with apparent positive connotations, but reinforcing gender stereotypes, pointing to socially defined functions for women. For lesbians, the results center the content on the prejudice suffered by them as members of the LGBT community. (AU)


El objetivo de esta investigación fue identificar los elementos que componen las representaciones sociales de estudiantes universitarios sobre la participación de las mujeres en el contexto laboral, considerando la orientación sexual de dichas mujeres. Con este fin, 177 estudiantes respondieron a la Prueba de Asociación Libre de Palabras, con los términos inductivos "mujeres en el contexto laboral" y "lesbianas en el contexto laboral". Los datos se analizaron mediante análisis prototípico, realizado por el programa Iramuteq. Para las mujeres en general, los resultados indican el uso de palabras con connotaciones aparentemente positivas, pero que refuerzan los estereotipos de género, señalando funciones socialmente definidas para las mujeres. ABSTRACT Objectives: to perform a systematic review and meta-synthesis of qualitative studies about the work-related quality of life of Family Health Strategy professionals. Methods: this systematic review was developed to answer the following PVO question: "Which factors (variables) are associated with the work-related quality of life (outcome) of Family Health Strategy professionals (population)?" The PubMed, Scopus, Embase, SciELO, Web of Science, LILACS, Science Direct, OpenThesis, OpenGrey, and OATD databases were selected. The meta-synthesis analyzed the main codes and secondary codes of all included studies. Results: the database search resulted in 1,744 reports; six were considered eligible for the meta-synthesis. Four factors were considered for the quality of work life: working conditions; work processes; interpersonal relationships; and personal aspects. Conclusions: although this study confirms the adequacy of aspects commonly related to the quality of work life, other factors are important in the case of FHS professionals, especially work context.


RESUMEN Objetivos: desarrollar una revisión sistemática y metaanálisis de estudios cualitativos sobre la calidad de vida relacionada al trabajo de profesionales de Salud de la Familia. Métodos: la revisión sistemática se desarrolló para responder la siguiente pregunta pautada en metodología PVO: "¿Cuáles factores (variables) están asociados con la calidad de vida relacionada al trabajo (resultado) de profesionales de Estrategia Salud de la Familia (población)?". Fueron seleccionadas las bases PubMed, Scopus, Embase, SciELO, Web of Science, LILACS, Science Direct, OpenThesis, OpenGrey y OATD. La metasíntesis analizó las categorías principales y subcategorías de todos los estudios incluidos. Resultados: la búsqueda en bases halló 1.744 registros, seis de ellos fueron considerados elegibles para la calidad de vida relacionada al trabajo: condiciones laborales, proceso de trabajo, relaciones interpersonales y aspectos personales. Conclusiones: Aunque este estudio confirme que algunos aspectos habitualmente relacionados con la calidad de vida son adecuados, hay otros factores importantes para los profesionales de Salud de la Familia, particularmente el ámbito laboral.

Quatro fatores foram considerados para a qualidade de vida relacionada ao trabalho: condições de trabalho, processo de trabalho, relações interpessoais e aspectos pessoais. Conclus ões : embora este estudo confirme a adequação de aspectos comumente relacionados à qualidade de vida no trabalho, outros fatores são importantes no caso dos profissionais da Saúde da Família, especialmente o contexto de trabalho.

ABSTRACT

We performed a scientometry to validate trends in the scientific production on phytotelmata and the importance of these microenvironments in the maintenance of biodiversity. We searched for articles in the Web of Science and looked at publications from 1987 to 2016. We collected years of publication, surveyed organisms, countries where the surveys took place, plants that accumulate phytotelmata, scientific journals that publish more about the topic and their respective IFs (Impact Factor) and the keywords of each article. We built a heatmap using the most frequent keywords in the studies, to investigate the topics studied over the years. We have found 293 publications, increasing over the years. Insects, anurans and crustaceans were the most studied organisms. The most studied plant families were Bromeliaceae, Poaceae and Apiaceae. The published studies analized were carried out mainly in Brazil, Argentina and Peru. The most published journals on the subject are: Zootaxa, Hydrobiologia, Biotropica and Journal of Natural History. Some main global concerns such as climate change, habitat fragmentation has gained the attention of the phytotelmata studies in the recent years. The results contribute to the knowledge about phytotelmata accumulated biodiversity and research trends.


Realizamos uma cienciometria para demonstrar tendências na produção científica sobre fitotelmata e a importância desses microambientes na manutenção da biodiversidade. Buscamos artigos no Web of Science e analisamos publicações de 1987 até 2016. Coletamos os anos de publicação, organismos estudados, países onde ocorreram as pesquisas, plantas que acumulam fitotelmata, revistas científicas que mais publicam sobre o tema e seus respectivos FIs (Fator de Impacto) e as palavras-chave de cada artigo. Fizemos um heatmap, utilizando as palavras-chave mais frequentes nos estudos, para investigar os tópicos estudados ao longo dos anos. Encontramos 293 publicações, com aumento ao longo dos anos. Os insetos, anuros e crustáceos foram os organismos mais estudados. As famílias de plantas mais estudadas foram Bromeliaceae, Poaceae e Apiaceae. As pesquisas foram realizadas, principalmente, no Brasil, na Argentina e no Peru. Os periódicos que mais publicam sobre o tema são: Zootaxa, Hydrobiologia, Biotropica e Journal of Natural History. Os resultados contribuem para o conhecimento sobre a biodiversidade acumulada em fitotelmata e tendências nas pesquisas.

Abstract: Blue nevi are benign melanocytic lesions located in the deeper reticular dermis, consequence of failure of melanocytic migration into the dermal-epidermal junction from the neural crest. Lesions are usually asymptomatic and solitary, but may present in a multiple or agminated (grouped) pattern. The agminated subtype is formed when bluish-pigmented lesions cluster together in a well-defined area. Lesions can be flat or raised. We report the case of a patient who presented multiple bluish macules (1-3 mm in diameter) grouped on the left upper back. Dermoscopy and anatomic pathological examination were consistent with blue nevus.

ABSTRACT

Abstract: Kaposi's sarcoma (KS) is a multicentric vascular neoplasm, with cutaneous and extracutaneous involvement. Different clinical and epidemiological variants have been identified. The classic form is manifested mainly in elderly men with indolent and long-term evolution, with lesions localized primarily in the lower extremities. We present two cases of classic Kaposi's sarcoma (CKS) in two female patients with extensive, exuberant skin involvement and rapid evolution, with good response to radiotherapy.

ABSTRACT

Abstract

Collision tumors are characterized by the coexistence of two cancers in the same anatomical site and its pathogenesis remains controversial. Although uncommon, the association of basal cell carcinoma and melanocytic nevus is the most common among combinations of skin tumors. Even rarer is the association of two malignant tumors. We report a case of tumor collision representing melanoma and basosquamous cell carcinoma, a combination not previously described in the literature, since there are no reported cases of melanoma with this type of basal cell carcinoma.

Abstract

Darier's disease is a rare autosomal dominant genodermatosis. It has an estimated prevalence of 1 in 55,000 to 100,000 individuals, regardless of gender. It is characterized by multiple keratotic papules on the seborrheic areas of the trunk, scalp, forehead and flexures, and the clinical picture is worsened by heat, sun exposure, perspiration and mechanical trauma. Histopathology observed loss of epithelial adhesion and abnormal keratinization. About 10% of cases present in the localized form of the disease. We report a case of segmental Darier's Disease Type I and discuss the main characteristics of this condition.

Introdução:A cobertura vacinal pode ser determinada por três métodos distintos: administrativo, inquérito e o Monitoramento Rápido de Cobertura (MRC). No Brasil, o método mais utilizado é o administrativo. Porém, considerando-se a heterogeneidade nos resultados, o Ministério da Saúde institucionalizou o MRC como a principal ferramenta para verificação rotineira da situação vacinal. Este estudo avaliou se as coberturas para tetravalente, febre amarela e influenza estavam de acordo com o preconizado pelo Programa Nacional de Imunizações a partir da realização de um MRC. Trata-se de um estudo populacional descritivo. Métodos:O cálculo amostral baseou-se no Protocolo de MRC do Ministério da Saúde e constituiu-se de crianças de 1 a 2 anos, residentes em áreas de abrangência de duas unidades de saúde do bairro Célvia, em Vespasiano. Resultados:A cobertura encontrada para febre amarela e tetravalente no Célvia 1 foi de 100% e no Célvia 2 de 96%. Em relação à influenza, a cobertura variou de 68% no Célvia 1 a 75% no Célvia 2. Não houve diferença estatística significativa entre os valores, com p variando de 0,754 a 1,000. Conclusão:As coberturas obtidas pelo Monitoramento Rápido de Cobertura para as vacinas febre amarela e tetravalente em ambas as áreas estudadas corresponderam à meta operacional preconizada pelo PNI (95%). Em relação à vacina Influenza, observou-se cobertura vacinal abaixo do recomendado (80%) nas duas unidades de saúde. Embora não seja possível fazer uma comparação estatística, observou-se diferença entre a cobertura administrativa e o MRC em relação à vacina Influenza.


Introduction:Vaccination coverage can be determined through three different methods: Administrative, Baseline Study and rapid Monitoring of Vaccination Coverage (MVC). From those, Administrative method is the most used in Brazil. However, due to heterogeneity in vaccination coverage results, MVC was recommended by the Ministry of Health as a supervisory tool to verify vaccination status. This population-based descriptive study aimed at evaluating if vaccination coverage for tetravalent, yellow fever and influenza vaccines were in accordance with Brazil's National Immunization Program (NIP) recommendations when MVC was used. Method:Sample calculation, having the MVC Protocol from the Ministry of Health as a basis, included children aged 1 to 2 years old, who lived in areas covered by two different outpatient care units (Célvia 1 and Célvia 2) in Célvia neighborhood, Vespasiano, Brazil. Results:Yellow fever and MMR vaccines coverage was 100% at Célvia 1 and 96% at Célvia 2. Influenza coverage has varied from 68% at Célvia 1 to 75% at Célvia 2. Those values showed no significant statistical difference, with p-value varying from 0.754 to 1.000. Conclusion:Use of MVC at Célvia 1 and Célvia 2 showed that yellow fever and tetravalent vaccines coverage reached NIP target (95%). A different result was shown for influenza vaccine, whose coverage was under the recommended target (80%) in both outpatient care units. A difference between administrative coverage and MVC was found, although a statistical comparison is not possible.

ABSTRACT

BACKGROUND: Changes arising from the aging process, particularly changes in body composition, contribute to the functional decline of the elderly. OBJECTIVE: To compare the body composition and muscle strength, mobility and quality in active elderly women according to the distance walked during the 6-minute walk test (6MWT). METHOD: The study included 77 active elderly women aged 65 to 80 years, who were divided into tertiles (A, B and C) according to the distance covered in the 6MWT. We performed anthropometric and clinical evaluations. Body composition was determined by dual energy X-ray absorptiometry (DXA). Handgrip strength (HGS) was measured with a portable dynamometer (Saehan), and knee extension strength (KES) was measured with the one repetition maximum test (1-RM). Functional mobility was assessed by the Timed Up and Go (TUG) test, and body balance was assessed by the Berg Balance Scale (BBS). Muscle quality was defined by the ratio between muscle strength (kgf) and muscle mass (kg). RESULTS: The group that walked the shortest distance in the 6MWT had a higher BMI (A=30.8±7.0, B=27.2±4.2 and C=25.9±3.5 kg/m2), greater amount of fat mass (A=31.3±10.7, B=25.9±6.7 and C=23.81±6.46 kg) lower HGS (A=21.8±5.1, B=22.1±3.5 and C=25.5±5.1 kgf), lower knee extension strength (A=30.6±10.9, B=40.4±12.5 and C=47.2±10.1 kgf), lower arm muscle quality (A=10.1±3.7, B=11.6±2 and C=12.7±2.2 kg) and lower leg muscle quality (A=1.78±1, B=2.84±0.98 and C=3.31±0.77 kg). There was no significant difference between muscle mass (p=0.25) and lean mass (p=0.26). CONCLUSION: Body fat has a negative influence on functional performance, even among active elderly women. .

ABSTRACT

Os brócolis contêm quantidades relevantes de vitaminas e minerais e, nestes vegetais in natura os teores de potássio e vitamina C são >30 por cento da IDR (ingestão diária recomendada). As formas de transferência de calor, intensidade da temperatura, duração do processo e meio de cocção utilizados são fatores que acarretam alterações nas características químicas, físicas e nutricionais do alimento. Neste estudo os teores de umidade, vitamina C, potássio, cálcio e fósforo foram avaliados em brócolis submetidos a diferentes técnicas de cocção (panela a vapor, forno combinado, forno de micro-ondas, ebulição e fogo brando). Nos brócolis in natura os teores de potássio e vitamina C foram, respectivamente, de 229,23 ± 7,88 e 50,79 ±6,55 mg/100 g. Comparando-se esses teores com aqueles obtidos pelas técnicas de panela a vapor e forno combinado não houve diferenças significativas, ao contrário dos resultados observados para as demais metodologias. Houve perdas significativas de teores de fósforo e cálcio em todas as técnicas de cocção, em comparação aos brócolis in natura. A cocção na panela a vapor e no forno combinado foram as técnicas que preservaram mais adequadamente os teores de vitamina C e potássio. A retenção de nutrientes é o fator preponderante na indicação de procedimentos de cocção.

ABSTRACT

O objetivo deste relato é alertar a comunidade médica sobre o escorbuto que, apesar de ser doença rara nos dias atuais, deve ser lembrado quando o paciente manifesta distúrbios hemorrágicos e história de erro alimentar. Reporta-se o caso de uma criança que foi hospitalizada por dor progressiva nos membros inferiores, edema e manifestaçõeshemorrágicas. O diagnóstico de escorbuto foi baseado nos achados clínicos, no relato histórico alimentar de ingestão somente de bolo e leite e na remissão dos sinais e sintomas após a administração de vitamina C.


The purpose of this report is to alert the medical community about scurvy. Although it is nowadays a rare disease, it must be taken into consideration when the patient presents bleeding disorders and bad eating habits. A case is presented of a child hospitalized with progressive pain in his lower limbs, edema and hemorrhages. The diagnosis of scurvy was based on clinical findings, on the alimentary history of only cake and milk intake and, on the remission of symptoms and signs after the administration of vitamin C.

ABSTRACT

OBJETIVO: Chamar a atenção para as complicações, que podem surgir em qualquer fase da Doença de Kawasaki (DK), para os fatores de risco que contribuem para o aparecimento dessas complicações e para as possíveis sequelas da doença, sejam elas transitórias ou permanentes. MÉTODOS: Estudo prospectivo (coorte clínica) realizado entre abril de 2002 e abril de 2009 de 115 pacientes com DK internados no serviço de Reumatologia Pediátrica do Hospital Geral do Distrito Federal. Todos os pacientes foram sequencialmente avaliados com exames clínicos e laboratoriais, ecocardiogramas com Doppler, imitanciometria, potenciais evocados auditivos, avaliação psicológica, exame oftalmológico e, em um paciente com coreia, angiorressonância magnética cerebral. Em todos os pacientes foram aplicados questionários avaliando a possível presença de dificuldades cognitivas, emocionais, comportamentais e sociais. RESULTADOS: Vinte e cinco pacientes (21,7 por cento) apresentaram aneurismas de coronárias. Trinta e oito pacientes (33 por cento) apresentaram perda auditiva neurossensorial durante a doença aguda e subaguda, e 13 pacientes (11,3 por cento) mantiveram a perda auditiva seis meses após a primeira avaliação. Outras complicações observadas foram: paralisia facial em um paciente (0,9 por cento), ataxia na fase aguda e subaguda em 11 pacientes (9,5 por cento), complicações oftalmológicas em 15 pacientes (13,2 por cento), constatando-se uveíte em 13, edema de papila em um paciente e hemorragia conjuntival em outro. Um paciente apresentou coreia (0,9 por cento) sendo que a angioressonância magnética evidenciou alterações compatíveis com isquemia cerebral. Em um paciente constatou-se a presença de aneurisma de aorta torácica (0,9 por cento), e outro apresentou vasculite necrosante que evoluiu com gangrena periférica e perda da ponta da língua (0,9 por cento). Alterações de comportamento durante a convalescença (20 por cento) foi observada em 23 crianças. CONCLUSÃO: A DK pode evoluir com complicações diversas, mesmo meses após a fase aguda da doença, eventualmente resultando em sequelas permanentes. Quanto mais precoce forem o diagnóstico e a intervenção terapêutica com a administração de IgG IV, menor será a ocorrência de complicações. Presença de trombocitose, anemia e de atividade inflamatória elevada e por tempo prolongado são fatores de risco para o aparecimento de complicações.


OBJECTIVE: To draw attention to complications that might arise in any Kawasaki disease (KD) stage, risk factors contributing to the onset of complications and possible transient or permanent disease sequelae. METHODS: Prospective study (clinical cohort) conducted between April 2002 and April 2009 of 115 patients with KD admitted to the Pediatric Rheumatology Clinic of the General Hospital of the Federal District, Brazil. All patients were sequentially assessed with clinical and laboratory examinations, Doppler echocardiography, imitanciometry, auditory evoked potentials, psychological evaluation, ophthalmologic examination and, in one patient with chorea, cerebral magnetic resonance angiography. In all patients, a questionnaire assessing the possible presence of cognitive, emotional, behavioral and social disorders was applied. RESULTS: Twenty-five patients (21.7 percent) had coronary aneurisms. Thirty eight patients (33 percent) had a sensorineural auditory loss during the acute and subacute phases of the disease and 13 patients (11.3 percent) maintained the auditory loss six months after the first assessment. Other complications observed were as follows: facial palsy in one patient (0.9 percent), ataxia in acute and subacute phases in 11 (9.5 percent); 15 patients had ophthalmologic complications (13.2 percent), with uveitis in 13, papilledema in one patient, and conjunctival hemorrhage in another patient. One patient experienced chorea (0.9 percent), with a magnetic resonance angiography showing changes consistent with cerebral ischemia. In one patient, a thoracic aorta aneurism was found (0.9 percent) and another patient had a necrotizing vasculitis progressing to peripheral gangrene and tongue tip loss (0.9 percent). Behavioral changes over convalescence were observed in 23 children. CONCLUSION: KD may progress with several complications even within months of the disease acute phase, eventually resulting in permanent sequelae. The earlier the diagnosis and therapeutic intervention with IV IgG administration are, the lower will be the occurrence of complications; the presence of thrombocytosis, anemia and elevated and extended inflammatory activity are risk factors for complication arising.

ABSTRACT

Esta pesquisa, realizada com agentes comunitários de saúde (ACS), objetivou: (1) avaliar a saúde mental em ACS; (2) descrever os eventos vitais vivenciados no último ano por ACS; (3) averiguar o nível de ansiedade apresentado por ACS; (4) correlacionar saúde mental, ansiedade e eventos vitais em ACS. Participaram 116 ACS. Instrumentos: questionário sociodemográfico, Questionário de Saúde Geral de Goldberg, Escala de Eventos Vitais, Inventário de Ansiedade Traço-Estado. A maioria dos ACS apresentou grau de ansiedade moderado e saúde mental positiva e relatou ter vivenciado poucos eventos estressantes no último ano, embora a maioria tenha tido problemas de saúde. O fato de estarem presentes níveis de ansiedade, mesmo moderados, sugere uma provável interferência de características específicas da profissão. Entretanto, o fato de a pessoa identificar subjetivamente a ansiedade e classificá-la em um determinado nível pressupõe que esteja usando estratégias de controle, fato esse que auxilia a manter a saúde mental.


The scope of this research, carried out together with community-based health workers (ACS) was: (1) to evaluate the mental health of the ACS; (2) to describe the vital events in the last year of the ACS; (3) to inquire about the level of anxiety of the ACS; (4) to correlate mental health, vital events and anxiety of the ACS. Participants: 116 ACS. Measurements: socio-demographic questionnaire, Goldberg General Health Questionnaire, The Life Events Inventory, State-Trait Anxiety Inventory. The majority of the ACS presented a moderate degree of anxiety and positive mental health; they reported having experienced few stressful events in the past year, even though the majority had experienced health problems. The fact that most of the ACS reported having experienced moderate anxiety levels suggests a probable interference of specific characteristics of the profession. However, the fact that the persons subjectively identified the presence of anxiety and classified it at a given level presupposes that they are using control strategies, which is a factor that assists in maintaining mental health.

ABSTRACT

Avaliar o equilíbrio postural e o risco de quedas em idosos institucionalizados e não-institucionalizados. Métodos: Foram incluídos no estudo 45 idosos institucionalizados e 43 idosos não-institucionalizados. O equilíbrio e o risco de quedas foram avaliados através da Escala de Equilíbrio de Berg. A comparação do equilíbrio entre os grupos foi feita por meio do teste de Mann-Whitney. Foi avaliado o risco para queda entre as populações. Adotou-se um valor de p 0,05 como significante. Resultados: Houve diferença entre os escores da Escala de Berg nas populações estudadas (p < 0,0001). Os resultados mostraram que a população institucionalizada apresenta nove vezes mais risco de cair do que idosos da comunidade (p=0,0001). Conclusões: Os resultados indicam que o equilíbrio postural é menor na população institucionalizada, aumentando seu risco de cair.
